REFERRAL PROCESS OVERVIEW
1. Referral is received 2. Referral form is completed.2. Referral is scheduled for assessment and intake within 48 hours.
3.Appointment is logged in calendar.4. Once assessment and intake are completed and received, CPSS supervisor and staff will be assigned to member.5. CPSS Supervisor will make an introduction call to introduce themselves, gather additional information, and provide member with the name of the CPSS assigned to them.6. CPSS will call and schedule first visit to introduce themselves and begin collecting information to use to develop member’s person centered plan.
7. The information collected will be given to the PSS supervisor to assist in creating the member’s PCP.
